Upcoming events: 'Home at the heart of Intergration' and  'Transforming the Discharge Pathway '

'Why am I still here? Transforming the Discharge Pathway' 

  The Improvement Network for Integrated Care and Support is delighted to announce a collaborative learning event to help you to improve the outcomes and experience for people returning home or moving on from hospital.

 You will share learning about new ways of working that reduce the days people spend in hospital when they are ready to be discharged and explore what more you need to do to increase the percentage of people who are discharged from hospital within 72 hours of being ready – the new integration indicator.       

 The day will be of interest to health and social care practitioners and managers who lead on capacity, flow and discharge within hospitals, colleagues from community services and partners from housing, Third sector and independent sector.

 Interactive breakout sessions will include examples of innovation and good practice on:   

  • Discharge planning  

  • Triage and streaming to Discharge Flow Groups

  • ‘Discharge to assess’ pathways

  • Updated guidance for reporting on delays

  • Using Ward View for managing discharge

  • Discharge Hubs – the next generation – and Single Point of Access

  • AWI and Choice / Moving on guidance

  • Power of Attorney campaign

  • Hospital to home support from Third sector and transport partners

  • Access to housing and equipment / adaptation  

This event will take place on Thursday Oct 1st   at James Watt Centre, Herriot Watt University, Edinburgh EH14 4AS.

‘Home’ at the Heart of Integration

A free event for everyone involved in planning, designing and delivering adaptations, with guest Speaker – Shona Robison, Cabinet Secretary for Health, Wellbeing and Sport.

 Objectives for the event are to

  • Demonstrate how adaptations contributes to delivering national health and wellbeing outcomes

  • Consider the role of housing and adaptations in the context of national programmes and priorities, e.g., delayed discharge.

  • Share the learning from the 5 adapting for change sites and other examples of good practice in the delivery of adaptations and housing solutions.

Who is the event for?

Managers/planners/ senior/specialist practitioners from health, social care and housing, the third and independent sectors engaged in planning, designing and delivering adaptations and equipment.
