Social Prescribing Project : Scotland and Northern Ireland

The Big Lottery UK funded Social Prescribing Project will bring together partners from across Scotland and Northern Ireland. It will produce learning from across 2 regions of the UK for 3 years. Over 16,000 people will co-create a social prescription and engage in interventions delivered by our network of experienced community led health organisations – allowing individuals to make decisions on their own health needs. 

The lead partner and grant holder is Bogside and Brandywell Health Forum BBHF (Derry, NI) with Scottish Communities for Health and Wellbeing (SCHW) as the main partner.

A Project Manager will lead (based in BBHF with 2 Regional Managers, one in Scotland and one in NI) who will support the partners. 10 community led health partner organisations in Scotland have been selected across rural and urban areas. Each partner will have a named ‘social prescriber’ who will be responsible for working with participants in the project co-producing a health pathway of activities and goals – giving participants control over their own journey. 

Progress will be monitored and recorded on an online system created by Elemental; The Social prescribing people. Elemental have designed a tailored online system where referral agents (such as GP’s, primary care staff) can refer directly to a social prescriber who will pick up the referral and make contact with the individual and arrange a time and place to meet that is suitable and comfortable for them. Using this system, meets the needs of all involved. The referral agent, the social prescriber and the individual can log in online and track the progress made.

This social prescribing project is the largest of its kind with co-production at the heart of the project. Working at this scale will build robust evidence of the impact of social prescribing and hopefully influence and shift the spend of health care.
